Blender3D is a freeware, multiplatform and open source 3D modeling (and more) tool.
You can download it from http://www.blender.org/download/
A (kind of) full manual -including how to install addons- can be read in https://www.blender.org/manual/
It's used by some modelers, like masisi, to make MMD models from scratch.
But some exterior extra tools are needed to make the transition between the two programs.
1) You'll need to install a blender addon (a plugin) to read and write MMD models.
There are three that I know. Choose the one you see to fit more your needs:

a) Nanashi's -I think- ( https://twitter.com/3dnanashi ) blender2pmx (there is an old pmd version, too).
This is the one masisi seems to use.

b) sugiany's mmd tools. This one is way more complex but it has more options.

There is one in there but the link is kind of hard to get.
I used to have this one but the new versions don't run on XP.

2) You may need to add a special touch to the model before it fully working with MMD (like adding the right IK properties to the bones) so you may need PMDEditor.
The Japanese versions can be download from:

(use 0.1.3.9 for old Windows XP or if 0.2.2.2 can't run because you don't have a 64 bit OS).
There are -partially?- translated versions of PMDEditor but as they're not official -I think- I suggest you to try with google -with your antivirus at alert!- if you need the translation. Sorry for the inconvenience.
Note: With these tools you can export your 3D models into MMD but you can also import from MMD models into blender as you can see in this video.
But be careful with testing models in blender as some modelers don't allow the use of the characters outside MMD.
