To make your own index.csv, format it like this:
filename.wav,phoneme_phoneme_phoneme_phoneme

For example
annyeong.wav,a_ny_yeo_ng
butts.wav,i_like_big_butts

These will get generated into aliases like this:
[- a][a ny][ny yeo][yeo ng][ng -]
[- i][ i like][like big][big butts][butts -]

As for replace.csv, it's like this.
original alias,new alias

So you could make a replace.csv like this
ny yeo,녀
big butts,BOOTEH

I made an index and replace for japanese, so you can look at those for an example.
